This position is caring for patients mostly in the Skilled Nursing Facility setting which requires understanding of dynamics within the facilities, communicating with both patients, families, and facility staff in an effective manner, participating in facility IDG and case conference meetings, and providing all necessary documentation to facility in a timely manner.
The Hospice RN Case Manager serves as case manager for the IDT and is responsible for the delivery of quality hospice nursing care for each patient assigned.
Essential Functions Provides hospice nursing care to patients and families utilizing current principles of palliative care and symptom management to produce hospice outcomes.
Coordinates care in a cost efficient manner.
Collaborates effectively with other members of IDT.
Utilizes nursing process and current standards of nursing practice in providing hospice care.
Remains competent on technical nursing skills (i.
e.
IVs, Gtubes, etc.
) Provides effective patient and family teaching.
Communicates with physician and VNA staff regarding changes in patient's condition.
Understands family dynamics and works effectively within various types of family systems.
Demonstrates ability to act as a team player.
Provides peers with support and mentoring if appropriate.
Utilizes resources appropriately.
Demonstrates the ability to act as a patient and family advocate.
Provides information to families about all available resources.
Accurately addresses need for additional services.
Performs adequate number of nursing visits necessary to provide superior hospice care under MVH standards.
All aspects of patient care is communicated and documented.
Communicates patient reports and status changes concisely and effectively.
Performs and documents HHA supervision every 14 days.
Updates Care Plan, Medication Profile and HHA Treatment Plan routinely and as patient condition changes.
Completes all nursing documentation per procedures and nursing standard of practice including assessments, interventions, responses to interventions, communications, verbal orders, etc.
Performs in a professional manner at all times.
Manages caseload efficiently and effectively.
Involves Clinical Manager and Medical Director in situations appropriately.
Follows guidelines for weekend and evening coverage.
Understands how to maintain professional boundaries with patients and families.
Utilizes good customer service skills internally and externally.
Assists in hospice education in community.
Position Type/Expected Hours of Work & TravelThis is a full-time position, and days and hours of work are Monday through Friday, 8:
00 a.
m.
through 4:
30 p.
m.
This position will also be required to work rotating weekend shifts every other weekend.
Qualifications Active RN Licensure in MA and NH required (or willingness to obtain dual licensure) Interest in end-of-life care Recommended Skills Assessments Certified Nurse Practitioner Clinical Works Coaching And Mentoring Communication Customer Service Estimated Salary: $20 to $28 per hour based on qualifications.
