9 Lawrence, KS Lawrence, KS Full-time Full-time $17 - $21 an hour $17 - $21 an hour 14 days ago 14 days ago 14 days ago Welcoming all who need care Job title Care Coordinator Reports to Behavioral Health Lead Status Non-Exempt (Hourly) Job purpose The Care Coordinator is a member of the interdisciplinary medical team of the health center, providing support to the Heartland Community Health Center (HCHC) Behavioral Health Consultant (BHC) providers.
This position assists with direct and indirect patient care, including returning patient calls, care coordination, screening for suicidal ideation and follow up, setting appointments, following up with missed appointments and limited care coordinator duties.
Duties and responsibilities Helps build a strong team Participates in staff meetings and team problem-solving Works as an extension to BHC services Schedules BHC appointments and returns patient calls in collaboration with BHC's Provides follow up contacts and calls to assure that patients are receiving critical services in collaboration with BHCs Evaluates and refers individuals that present in crises to the clinic in collaboration with BHCs Assures that resource documents (housing, food, etc.
) are up to date for patient, HCHC staff and BHC use Assists patients in identifying available resources in the community, advocating for access to care, and identifying natural supports Reinforces BHC treatment plan through contact with patients, information and feedback regarding BHC recommendations for symptom management Works to increase patient understanding of health condition/health literacy and motivation to engage in self-management through both individual and group contact Assist BHCs with required paperwork Performs other related duties as assigned or requested Qualifications High School graduate or equivalent required Bachelor's Degree in psychology, social work or related field is preferred and/or experience in related services Ability to learn/navigate computer systems such as an electronic health record system and Microsoft Office; generally technology-savvy Strong verbal communications skills and ability to communicate ideas successfully Works in harmony with others, exhibits a positive attitude and commitment to the health center mission.

30 am - 7 pm as needed.

Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
