M-F 8:
30AM-5PMThe Intake Coordinator is responsible for, but not limited to:
Completes initial inquiry forms for all potential admissions, and forwards completed forms to the appropriate Hospice Nurse and Director of Clinical Services and/or AdministratorInsures that whenever possible, all referrals are converted to admissionsIf referrals do not convert to admissions, the Administrator will be notifiedTrack all inquiries and referrals daily.
This report will be given to the Administrator weeklyAssists patients, families and responsible parties in completing the admissions packet as neededWorks collaboratively with appropriate staff to insure written or verbal consents are obtainedEducation:
LPN not required, but preferred.
Professional
Experience:
Preferably 5-8 yearsExperience in a related capacityMust have a Valid Driver's License and reliable source of transportationMust be willing to travelExcellent communication, interpersonal and organizational skillsAscend Hospice - is part of the Ascend family of Healthcare Companies.
